try to disable your mod securty on the server if you can that can sometime give you this issue and can u add an image to this just i case i done have all the answer to your issue and that othere members might have ran into the same thing as you better image -

try to upload it from your webserver and not ftp see what happens -
you can do this when you creat the form just put your own image
