For me, the error was a duplicate ID in the database for members and member groups.

If anyone is any good with .tar.gz files and php some help might be appreciated, I have the files fixed but for some reason, the installer isn't recognising one of the php class files. Ill continue to to work on this folks! :)

It's called the white page of death and it's quite common, not sure if 4.0 has it but if it does copy upgradeFinish/index.php over to: /admin/upgradeFinish/index.php then navigate to this in your browser and run it, should fix that problem. :)
